Crater District Area Agency on Aging has Case Managers and an Information and Referral Specialist available during regular office hours to respond to requests for information and assistance. Our staff can provide information relevant to services of interest to seniors, including long term care options and caregiver assistance. The staff performs the following services:

• Outreach efforts to educate the community about agency and community services.
• Evaluates needs through an intake or screening process.
• Performs comprehensive assessments when needed to identify needs beyond the presenting problem.
• Develops a care plan to address needs
• Coordinates services, makes community referrals and performs other collaborative duties on behalf of the family.
• Monitors service delivery and reassesses as needed.
